Concordia Health Plan members can sign up for a free Vitality Check (biometric screening) through Quest labs. Getting a screening will help participants better understand their health status, present a more accurate Vitality age, and reward them with Vitality points. Screening results will be automatically uploaded into the Vitality platform in 10-14 business days.
Members can schedule a Quest screening onsite (if available) or at a Quest lab by following the steps below. They can also watch a video tutorial at ConcordiaPlans.org/screening to walk through the steps of setting up an appointment.
Sign in to Vitality through BeWellServeWell.com. Click the Learning Center tab on the top right, then Guide to Vitality > Vitality Points > Vitality Check > Quest. Accept the terms and conditions on the Quest website and enter your remaining personal information to register. Click on Get started > Schedule now > At event or At a Patient Service Center. Then choose your location and select the time for your screening.
In order to get accurate readings, you’ll need to fast (no food or beverages beside water) for 8-10 hours prior to your screening. But be sure to drink plenty of water. If you have questions or need help scheduling your screening, call the Quest support line at 1-855-623-9355.
Members have the opportunity to get $45 off of a fitness device purchased through Vitality. This include various models and colors of Fitbit, Polar, and Garmin devices. Sign in to Vitality through BeWellServeWell.com. Click the Rewards tab from the top menu bar, then click Fitness Devices. Choose the one you want and click to “BUY” and Checkout.
This will take you to your cart. On the right side there is an option to apply a promo code or discount. It should be pre-filled with “BeWellServeWell.” Click apply and it will take $45 off your device. Then continue the checkout for the rest of the purchase.

For years, we’ve been hearing from our members that they want a subsidy for their gym membership. In the past, we have not been able to facilitate this at a national level. However, through Vitality, we will be able to offer a subsidy for paying gym members. Details on the exact amount of the subsidy and how it will be implemented will be coming in the later part of 2016. For now, if you pay for a gym membership, hang on to your proof of payment.
Members have the opportunity to plan their points pathway by clicking the center Points tabs and then the drop down Points Planner. Below will appear categories that you can engage in and put on your pathway to attain a higher status. Categories such as Online Education, Goals, Prevention, etc… to choose from. Once you add the category to your pathway, an icon will appear (below) and when you hover over it, it will outline all the categories you have added to your pathway. Don’t forget to check in weekly if you set goals as you will receive 30 points each week just for checking in!!
